Hazing Report Form


Stanford takes violations of the hazing policy extremely seriously, and encourages anyone who has been hazed, has witnessed hazing, or suspects someone they know has been hazed, to feel empowered to report these observations to university officials.

Hazing includes any activity done in connection with a sports team or student organization, regardless of whether the organization is officially recognized at Stanford, that causes or is reasonably likely to cause another student to suffer bodily danger, physical harm, or significant personal degradation or humiliation, even if no bodily danger, physical harm, or significant degradation or humiliation in fact results.

Reporter Information

Your information will be kept confidential - The University will make every attempt to safeguard the confidentiality of any report consistent with legal responsibilities and our need to review and investigate. The University will not share your information with the organization or team you submit a report about consistent with legal responsibilities.

You have the option to submit a report anonymously by not completing the Reporter Information section - Reporting anonymously limits the University's ability to investigate and respond to the alleged behavior(s). If you choose to report anonymously, please consider providing an anonymous method to contact you so that we may gather additional, crucial information.

University policy prohibits retaliation against individuals who raise concerns of perceived discrimination or harassment or who participate in the investigation of any claim of discrimination or harassment. Retaliation is any materially adverse action that would dissuade a reasonable person from making or supporting a claim of harassment or discrimination.
